If you're preparing for your child's first visit, you can do a few things to help expedite the appointment. Please provide the following information when you arrive for your visit:
- Insurance cards
- Complete immunization records
- Medical records from your previous pediatrician
Process for Transferring Care to Our Office:
Call our office at (574) 855-4575 to provide basic information on your child or children. If you need to request records from your current provider, please let us know and we will send you a records request form through the patient portal or you can print it below.
What Insurance Plans Do We Accept?
We accept a variety of commercial/employer-sponsored plans, including through the Health Marketplace. We do accept a variety of medicaid plans, as well. Please call the office for more information.
